The Central U.P. Youth Football League tracks all injuries for players.
Head Coaches and Parents can complete the following reports if applicable:

Insurance Claim Instructions
Question: When do I submit an insurance claim?
Answer: If your player is going to the hospital or is going to seek medical attention for his/her injury, you need to submit a claim.
The League's Insurance will cover expenses that your primary insurance did not cover. For example your deductible.
If an insurance claim is going to be filed the Head Coach must also complete PART1A - American Youth Football - Injury Report
American Youth Football - Injury Report
The claim will not be processed without Form Part 1A, so get the Head Coach to fill this form out ASAP!
Follow the procedures for submitting a claim.
